Step : 2 : Visit Your designated PSK with appointment letter along with the required original documents and xerox copies in person. you should be reach your designated PSK half an hour before on your appointment time provided by PSK.

(1) Birth certificates issued under the registration of births and deaths Act,1969
(2) Gas connection bill of one year before and current bill.
(3) Pension documents such as ex serviceman pension book/pension payment order , ex service men's widow/dependent certificates,old age pension order,widow pension order.
(4) Railway identity cards
(5) Property documents such as Pattas,Registered deeds etc.,
(6) Freedom fighter Identiy cards
(7) Electros Photo identity card
(8) Arms Liscenses
(9) Service photo identity card issued by state/central/government public sector undertakings,local bodies or public limited companies
(10) Ration cards
(11) SC/ST/OBC community certificates
(12) Photo copy of Diplomatic or official passport
(13) Proof of place of birth
(14) Affidavit stating the reason for change in date of birth
(15) Aadhaar card / letter
(16) Driving licenses valid and with in the jurisdiction of state of submission of application
(17) Income tax identity cards (PAN )
(18) Bank/kisan/Post office Passbooks
(19) Student photo identity cards issued by government recognized educational institutions in respect of full time courses.
